We're going to have one big imperial table for 46 guests.
Where do you sit bride and groom in Italy? and parents and witnesses?

the picture you see here shows what the etiquette requires. The bride sits to the right of the groom.Next to her, the groom's father, the maid of honor, the bride's father. On the other side, next to the groom, the bride' s mother, the best man, the groom' s mother.

Is there a way to get the NULLA OSTA without going personally to the US Embassy in Naples?

Dear Kelly,
unfortunately you must go there personally. This document includes a sworn declaration that must be signed by both applicants: bride & groom.
Call the Embassy and make an appointment for your convenince. Seven days before your wedding date will be fine.
